Health-check policy

Effective Date: June 1, 2021

• I have reviewed the COVID safety training available at this link: https://files.thrivedetect.com/navigate/file/cd2c48b1-18de-4e38-9d96-839ec65df68c
• I have either been tested (PCR) for COVID-19 and received a negative result within the last 7 days or are fully vaccinated as defined by the CDC.
• I have not had any signs or symptoms of a fever in the past 24 hours and I do not have any of the following symptoms currently:

• • Cough
  • Shortness of Breath or Chest Tightness
  • Headache
  • Difficulty Breathing
  • Sore Throat
  • Muscle Pain
  • New Loss of Taste and/or Smell
  • Vomiting

• For non-vaccinated individuals only: Within the last 14 days I have not been in contact with someone who has had a confirmed diagnosis of COVID-19
